A free wholesale order form template
The order form is the sheet a store buyer sends back to you. Each line is one style in one color, with how many of each size they want, and it adds itself up. Send it with your line sheet and the order comes back in a shape you can pack from.
Download the order form (.csv)
Opens in Excel, or in Google Sheets with File, Import, Upload; the units and totals add themselves up in both, with lines for shipping and a discount. Four lines are filled in so you can see the format (a jacket in two colors with a size run, a hat and a candle sold in case packs); delete them and add yours.
From the makers of Line Sheet Studio, an app for Shopify stores. The form is free and works the same wherever you sell; there's nothing to sign up for.
Fill in your part before you send it
- Your styles, colors and wholesale prices, one line for each style in each color. A buyer should only have to type quantities.
- Your sizes. Rename the size columns to the ones you make (0-2-4-6, 6-12M, S/M and L/XL) and delete the rest. No sizes? Use One size, or rename the columns to scents, lengths or pack sizes.
- Case packs, if you sell in them: how many come in one, so orders come back in whole packs.
- Your name and email at the top, and a reply-by date, so the form comes back to you in time.
- Your terms at the bottom. The form has a starting point to edit: "Opening order $300 minimum, reorders $150. Prepaid for a first order, then net 30. Anything damaged in transit is replaced if you send a photo within 7 days."
Not sure what to charge? Wholesale is often half of retail; the line sheet template walks through the price columns.
What the buyer fills in
- Store and buyer
- Who to invoice, and who to ask when something sells out before you ship. The email matters most.
- Ship to
- The store's receiving address, which isn't always the office.
- Ship window
- The first day they'll take the order and the day after which they can cancel it, as a range: Feb 1 to Mar 15.
- Quantities by size
- Units for each size in each color. If you sell in size runs or case packs, say so on the line ("Size run: 1 S, 2 M, 2 L, 1 XL") so the counts come back in whole runs.
- PO number
- The store's own reference. Put it on your invoice and on the box; it's how their back office matches the delivery.
- Resale or tax ID
- In the US, a store buying to resell gives you its resale certificate so you don't charge sales tax. Ask for it with a first order.
Before you say yes to an order
- Check it meets your minimum and every quantity is a whole size run or case pack.
- Check the ship window against what you have, or can make in time.
- Reply with the total, the ship date and the payment terms in writing. For a first order, ask for payment up front or a card on file.
Send it with a line sheet
Buyers order from a line sheet: the photos, the prices, the minimums. Our free line sheet template has the same styles and columns, so the two read as a set. Email both as attachments, or share the form as a Google Sheet a buyer can fill in on a phone.

On Shopify? Take orders from one link
Line Sheet Studio builds the line sheet from the products already in your store and lets buyers fill in an order by size and color from one link. On Pro, you can send the order into Shopify as a draft at your wholesale prices.
